If the consumer or the provider of IHSS (In-Home Supportive Services) have moved or has a plan of moving the consumer must notify the IHSS county office of the change of address. Below are the steps on how to change the address with IHSS. For more information and support dial
(866) 376-7066 - In order to change the address the consumer needs to download as well as fill SOC 840 form for IHSS Program Provider or the Recipient Change of Address
- This form allows confirming the current address, new home address, or a new contact phone number.
- The consumer can print out the form and hand write the answers or fill out the form online directly on the page
- The consumer must sign and date the form so as to ensure not to forget the important step if the consumer choose to fill out the form online
- Once the consumer has filled and signed the SOC 840 form the next step is to deliver it to the appropriate county office. If the consumer decides to mail the form, then it is advised to send it through certified mail. It is a good thing to notify the social worker of the change of address.
