2. Provide personal and basic information to receive an email with activation link.

4. Fill the "self-enrollment form at "https://borgesshealthcare.iqhealth.com/self-enroll/".
5. Patients need Patient Medical Record Number (MRN) to create an account.

Note: MyBorgessHealth patient portal is available for patients age 18 or older who don't currently have access to MyBorgessHealth platform.

2. Enter "Email address or username" and "Password".
